Cancellation policy
We understand that clients may sometimes need to amend or change appointments. If you are unable to make your appointment please let us know as soon as possible by email. We will try our best to be reasonable and accommodate the request. However, it would be at the Doctor’s discretion.
Clients can cancel or reschedule their appointment at no extra cost, provided that they notify us at least 72 hours in advance.
If you cancel your booking up to 48 hours before the appointment you will receive 75 percent fee and if you cancel with in 24 hours you will receive 50 percent refund and if you cancel on the same day (up to 4 hours before the appointment) you will get 25 percent refund. If you cancel with in 4 hours of the appointment you will not receive any refund. Please note this is to discourage DNA rates and to make sure the appointment not go unused when there are lot of people waiting for the appointment.
Confidentiality Policy
It is the our priority that information from our patients disclosed during your assessment including personal information, past and present medical history be kept secure and confidential.
However, our clinicians have duties bound by their own professional governing registration body’s policies on patient confidentiality. This means that when in certain, very specific and rare instances that can pose a threat of serious harm occur, they can be obliged to disclose significant information.
If you are initiated on medication treatment then we will be required to share relevant information with other relevant healthcare professionals, such as your GP, in accordance with the GMC guidelines on safe prescribing of medication.

We are not obliged to accept you as a patient if we judge that we are not the right service to support you. We do not have to give any reason why we may not accept you as a patient.
We will treat all patients with dignity, respect and without prejudice regardless of their age, sex, gender, or other protected characteristics.

The adultadhdandmentalhealth.com will perform a full assessment and may request investigations or assessments by other specialists, as required. We will not commence any treatment without reviewing the results of the required investigations, assessments, or specialist review.
Your Responsibilities
By booking an appointment with adultadhdandmentalhealth.com you agree to our terms and conditions.
Prior to the appointment, you must complete all sections of the patient registration form and/or other forms that you are requested to do so, to the best of your knowledge, to provide accurate, complete, up to date information.
The Patient Registration form and any other forms requested, must be sent back to us in a timely manner prior to your appointment. Your appointment may not go ahead or maybe delayed until this information is received from you, completed in full.
It is your responsibility to obtain any requested investigations or specialist review (which may incur a cost to you) and provide the results of these to adultadhdandmentalhealth.com in a timely manner. You understand that if you do not supply the results of any outstanding investigation or specialist review, at times these may lead to a delay in commencing your treatment or prevent us from commencing your treatment until your doctor believes that it is safe to do so.
You will be responsible for informing The adultadhdandmentalhealth.com of any changes in your medical health, mental health, or risk history or any significant changes in your personal and social circumstances. Should you withhold such relevant information, we cannot take any responsibility for any adverse effects on your physical or mental health.
You will notify us promptly in the event you need to cancel or reschedule any appointments that you have made, or we have made on your behalf.
Appointments are conducted online or face to face in our Cardiff Centre. You should arrive on time to your appointments whether remote on-line or Face to Face. Being late could result in your appointment being re-scheduled or cancelled without notice or refund; or the need for a second appointment if there is insufficient time to complete the consultation.

During your virtual appointment you should be in a quiet, private area where you can freely discuss your medical conditions privately without being disturbed. Your clinician has the right to cancel/reschedule your appointment without notice or refund if they feel that your privacy and confidentiality cannot be assured.

The adultadhdandmentalhealth.com is required to obtain your GP details prior to your assessment. These will only be used in emergency circumstances or when required by law. You are therefore required to provide details of your GP’s name, address and contact number on your registration form and these details must be always kept up to date. We will not use your data for any reason other than arranging your assessment, treatment on going care and for education and training purposes. There are some circumstances where we may be legally required to disclose your personal information, for example if you disclose information to us or our Consultant that you may pose either a serious risk to yourself or others, or information regarding acts of terrorism or a serious crime, or where we are legally required to in accordance with child protection legislation to report safeguarding/child protection concerns. By accepting these terms, you acknowledge that we are legally obliged to do this.

Retention of information

Whilst GDPR guidelines are clear that records should not be kept longer than is necessary, there may be reasons for retaining records. Zero Tolerance Policy
The adultadhdandmentalhealth.com is committed to providing a high quality of care to our patients. The welfare of patients and staff relies on establishing a good relationship between the patients and our staff. To ensure the safety and well-being of our staff and patients.

Patients will speak to our staff in an appropriate manner.
If our staff experience your communication as abusive then we may withdraw our services to you, or the patient on whose behalf you have been contacting us. Abuse in this context is not limited solely to inappropriate language but may include any form of communication which we judge to be unduly aggressive or critical of our services or staff. You will not be eligible for any refund relating to services already delivered. In the event that we judge your communication or behaviour as abusive and we decide to withdraw our services then where possible we will signpost you to an appropriate alternative provider where this is available, but we are under no obligation to do so.
In cases where the patient is intoxicated, or under the influence of drugs, we reserve the right to immediately terminate the appointment without notification or refund.
The adultadhdandmentalhealth.com has a zero-tolerance policy of such behaviour, particularly threatening or violent behaviour towards anyone associated with our Services.
The adultadhdandmentalhealth.com reserves the right to terminate an appointment (and call for police assistance, if deemed necessary) with anyone behaving in such a manner.
Disagreement with the diagnosis of our Consultant
Kindly note that many people now a days read or self-test online and that is not necessarily ADHD. The assessments are done to gather as much information as is possible. Whether or not a diagnosis of ADHD is given is based on all the information, if necessary QbCheck and DIVA-5, which is the NICE and UKAAN recommended diagnostic interview for Adults with ADHD. This means that following assessment, it is also possible that a diagnosis of ADHD is NOT given
At Adult ADHD and Mental Health, we understand that disagreements may occasionally occur regarding diagnoses. We encourage our clients to engage in open discussions about their concerns, as we value your perspective and experiences. However, it is essential to respect the views and expertise of our consultants, who base their assessments on thorough evaluations and established guidelines. By fostering a collaborative environment, we can work together to ensure the best possible outcomes for your mental health journey.
Emergency treatment or out of hours support/treatment
You should understand that Adult ADHD and Mental Health Clinic does not offer an emergency service and we are unable to treat patients in crisis, emergency, or high-risk situations within, or outside working hours.
If your mental health condition deteriorates, and you believe that you are in an emergency or crisis situation, e.g. having suicidal thoughts, considering harm to yourself, others, or to property, or if you are at risk from others, you should immediately inform a relative/carer and contact the local NHS Crisis Team, go to the nearest A&E, or call an Ambulance or Police on 999 or 111 press 2 (in Wales) for immediate support.
In certain situations we or our consultants may decide that you need a higher level or type of care, including more specialist services, than our consultants are able to provide in their private outpatient service. In that case we will refer you to an appropriate organisation accordingly.
NHS/GP Involvement
We will be unable to treat you without sharing information with your GP or other health practitioners / specialists as required.
We will be unable to start you on any relevant medication for ADHD without you undergoing some premedication baseline tests and clinical examination. Your psychiatrist will discuss these requirements with you.
We may also require a copy of your Summary Care Record from your GP before being able to start treatment.
If you are seeking an assessment privately and are hoping to return to your NHS GP for subsequent tests, monitoring, prescribing or any other procedure or service, then you are strongly advised to consult with your GP before being assessed by us to confirm with your GP to what extent they are willing to support you with this. GPs are not obliged to arrange subsequent tests, monitoring or enter into shared care agreements for prescribing and may refuse to do so. We cannot accept any liability in this eventuality. In the event that your GP is not willing to support you with these services then we would recommend that you arrange the services that you require through a private provider.
Prescriptions
Issuing a prescription is at the sole discretion of your psychiatrist following their consultation with you and is also subject to the limitations applied by Adult ADHD and Mental Health Clinic and good prescribing guidelines as stipulated by the GMC, NICE and in the BNF.
You will not be provided with a prescription for medication during your first assessment appointment. If medication is recommended, then you will only be issued with a prescription, if appropriate, after a subsequent follow up appointment and upon analysis of baseline investigations and receipt of your GP record. This is to ensure that safe prescribing practices are adhered to as stipulated in GMC, NICE and BNF guidelines.
Medications may have a risk of side effects, withdrawal symptoms and other possible complications.
You should be aware that your medication treatment may carry a risk of side-effects, withdrawal symptoms and possibly other complications. These effects may be heightened if you do not comply with, or adhere to, clinical instructions and/or if you exceed the recommended prescribed dosages of medication. If this were to continue adultadhdandmentalhealth.com may remove treatment without notice on the grounds of patient safety.
You should understand that it is also at your discretion to stop treatment or investigations, however, if you decide to do so against medical advice, you understand that this may carry serious risk of side-effects, withdrawal symptoms and/or other complications, for which you solely will take responsibility.
Please note that the cost of medication is not included in the cost of your assessment or follow-up appointments and is paid directly to the pharmacy.
If you are on a stable dose of medication and would like to request a repeat prescription outside of an appointment with your psychiatrist, then you will need to complete a repeat prescription form. A separate repeat prescription administration fee is payable. This fee must be paid before your psychiatrist is able to issue a prescription and this fee is separate to any additional costs from the pharmacy for the direct cost of the medications.
Delivery of medication prescriptions
At Adult ADHD and Mental Health, we aim to send all medication prescriptions via an online pharmacy for safety and delivery. Once handed over to the online pharmacy, the delivery of this prescription is not in our control, and Adult ADHD and Mental Health Clinic will not be held liable for any delays or issues locating prescriptions at the pharmacy.
In the event where a prescription is lost or damaged or the pharmacy is unable to issue the prescription for reasons such as the medication not being available, you must contact us immediately. You agree to destroy or return the un-used prescription immediately. You must provide Adult ADHD and Mental Health with photographic/video evidence showing the prescription being destroyed before a new one is issued.
If you want us to send the prescription to you directly instead of online pharmacy, we can do that. We will send in special delivery and cost would be covered by yourself (similar to online pharmacy prescription). If you want this, please speak to our Consultant during the appointment.
Pharmacy Charges
You will be charged by the pharmacy for the medication. The prescriptions available via Adult ADHD and Mental Health, are private prescriptions (not NHS prescriptions). You accept that private prescriptions are fulfilled at a charge, unique to the fulfilling pharmacy, which is not under the control of Adult ADHD and Mental Health Clinic. It is your choice as to whether you have any prescription fulfilled and at what cost. ‘Prescription’ also applies to repeat prescriptions. Controlled drug prescriptions are only valid for one month from the date of issue and cannot be used after this period.
Any prescription that you receive via us is only valid for use at legal UK pharmacies. You understand and agree that any prescriptions acquired from Adult ADHD and Mental Health Clinic are solely for your own personal use. Furthermore, you agree to read carefully all information provided to you and to follow the instructions provided by your psychiatrist on the prescription(s) itself (the label applied by the pharmacy) or otherwise, before taking any prescribed medicines. You also hereby agree to contact your psychiatrist or another doctor or pharmacist if you have any questions or do not fully understand the reasons you have been prescribed medication or the instructions related to the medication.
Shared Care
You understand that we cannot guarantee that your GP will being willing to enter into a Shared Care Agreement with Adult ADHD and Mental Health clinic. If you do go onto a Shared Care agreement then, you agree to attend 6 monthly review appointments with your Consultant Psychiatrist to monitor treatment response. Failure to regularly attend these appointments will result in the Shared Care Agreement being null and void and your discharge from the clinic.
Inactive Patient
You understand that if you do not attend an appointment for a period of 6 months and are on active treatment, then you will be discharged from the Adult ADHD and Mental Health.co.uk, and we will notify your GP.
Complaints
In the event that you need to make a complaint about any aspect of our service then you should communicate this to us. We will attempt to resolve it.
Liability
We are an advice and support service only. We do not accept any liability for any treatment that may be delivered by any clinician or Treatment Provider, partner organisation or other third party regardless of whether we have referred you to them. Contractual and tortious relationships for the purposes of treatment are between you and the clinician/Treatment Provider directly.
We shall have no liability to you for any failure of our obligations hereunder if we are prevented or delayed by any act or omission by you or a failure by you to comply with your obligations hereunder.
Nothing in these terms shall limit or exclude our liability for death or personal Injury caused by our negligence or any other liability which is not legally permissible to exclude. Subject to this we shall under no circumstances whatsoever be liable to you for any indirect or consequential loss and our total liability to you in respect of all other losses under these terms shall not exceed the fees paid by you in the year in which the loss arose.
Except as set out in these terms all warranties, conditions and other terms implied by statute or common law are, to the fullest extent permitted by law, excluded.
Miscellaneous
These terms constitute the entire agreement between us in relation to our advice and support service.
We shall not be liable to you as a result of any delay or failure to perform our obligations as a result of an event beyond our reasonable control including without limitation the act of God.
Any variation of these terms shall only be binding when agreed by us and sent to you by email. You cannot vary these terms.
You shall pay all amounts due without any deduction or withholding exception as required by law.
These terms and conditions are governed by and shall be construed in accordance with English law.
